Beginning this summer, all 7th through 12th grade Pirates participating in Athletics and Activities must have an updated Preparticipation Physical on file with the activities office before competing in any Hannibal School District summer camps, leagues, or clinics. To help with this new process, Advance Physical Therapy will be at Korf Gymnasium on May 17th from 3pm-5:30pm. Students must sign up for an appointment by contacting Advance Physical Therapy at 573-406-0576. *If a parent is unable to attend, students must bring the physical form with personal information and health history completed, before the physical can take place.*
Additionally, the PATCH center offers free physicals to district students during their regular business hours. Appointments can be made by calling 573-719-3247.
Important Notes:
- Physicals are required annually by the Hannibal School District. If the forms are not complete, students cannot participate in Athletics or Activities.
- MSHSAA updated their Preparticipation Physical Evaluation Forms / Physicals Requirement (effective April 2023), and any previous version of the form is void and cannot be used.
- ONLY the last page (Medical Eligibility Form - page 5) is to be returned to the school as the student’s documentation for receiving a physical examination and release to participate.
- If your student is getting a physical on May 17th at Korf, you will need to have both of the following forms completed: Preparticipation Physical Consent for Vision and Hearing Screenings.
